John David Smith, 45, passed away on July 1, 2014 at St. Vincent Mercy Medical Center. He was born on December 18, 1968 in Toledo, Ohio to John Paul Smith and Jimmie Siler. John is survived by his father, John; daughter, Allison and girlfriend, Amy. He was preceded in death by his mother. Services for John will be private.
